I am running P3Dv4.5 and GSX incl. level 2 (updated) with the QW787
The situation occurs on YSSY (scenery ORBX Australia v2)
I've been flying in and out YSSY several times, always parking at gate 57 (default). No problems with pushback whatsoever.
Until I decide to customize the parking with double gates with another logo.
Then....
The pushback starts and runs into an error popup, reading:
The addon <unknown> is causing the Couatl Scripting Engine to restart,
Please download and install the latest Stand-Alone Addon Manager from etc.
Well.. The SA Addon Man does not exist anymore as stated on the FSDT website.

So I deleted the yssy----.ini file from C:\Users\NAME\AppData\Roaming\Virtuali\GSX and tried again without customizing the gate 57. Same problem at pushback.
After this I (again) deleted the yssy----.ini file; at loading P3D I selected gate 9 and tried again. Same problem at pushback after customizing the gate.
Could this have anything to do with the fact that when customizing the gate, I loaded a jpg with the Sidney Airport logo? At various starts of P3D I've seen different other logo's all over the airport.
Would be nice if I can get the thing going again at YSSY.

By the way... after again deleting the ini file I restarted the PC, loaded P3D at the default P3D gate 57, and was able to do a normal pushback.
My guess is, whenever you customize something... better not do that.
